Calendar Year 1999 Premium Tax Package

**Use for Prior Year Amendments**

• Calendar Year 1999 Annual Premium Tax and Fees Report form Working Forms T-1 through T-8. Must be received on or before March 1, 2001. It is not necessary to return instruction pages. (12 pages, including instructions) 

• Calendar Year 2000 Quarterly Premium Tax Prepayment FormsWorking Form T-9. Due on or before April 15, June 15, September 15, and December 15. Use one form for each quarter as indicated on form. Companies with estimated current year premium tax liability of less than $500 may send forms and payment for all 4 quarters with the April 15 submission. Please indicate on form.

• Barcode Instructions - All companies must affix company identifier Bar Code to premium tax filings as instructed.  Barcodes can be generated using word processing software in a "3 of 9 Barcode Font".  This font is available online from various sources and can be downloaded and used to print barcode labels.  Carefully follow the instructions  that come with the "True Type Font" (ttf)  file.

• Authorization Agreement for Electronic Funds Transfer - The Delaware Insurance Department accepts ACH CREDIT (CCD+) transfers ONLY. This form must be received before electronic filing may commence.

  • Companies Currently Authorized: It is not necessary for companies already paying premium taxes via ACH Credit to complete this form unless there is a change in company address or contact person information.

  • Companies NOT Currently Authorized: Upon receipt of competed Authorization Agreement, file structure specifications and instructions will be mailed to the contact person listed on the form. NOTE: Selection of this option ill require your financial institution to originate a CCD+ credit NACHA format. Before committing to this option, please verify with your financial institution that they have the capability of originating a CCD+ format. Programming of file structure MUST be formatted to conform exactly to the specifications detailed in the instructions.
